Choose You offers an integrated approach to supporting residents in Bedford Borough, Central Bedfordshire and Milton Keynes to make long term improvements to their health and wellbeing. Whether they need support to lose weight or to stop smoking, our teams will guide them through the process.

How to refer someone to our services

Residents can sign themselves up for our service by completing our referral form, or you can submit a referral on their behalf. We will respond within five working days.

Who is eligible for Choose You services?

Our referral form is set up to ask the relevant questions for someone to find out if they are eligible for our enhanced services.

Below we have outlined the criteria for accessing our enhanced services for stop smoking, weight management, and family health programmes:

Stop smoking support

To qualify for stop smoking support, individuals must currently smoke or use smokeless tobacco and meet at least one of the following:

  • Live, work or study in Bedford Borough, Central Bedfordshire or Milton Keynes
  • Be registered with a GP in Bedford Borough, Central Bedfordshire or Milton Keynes.

Adult healthy weight programme

The eligibility criteria for adults are:

  • A BMI of 30+
  • A BMI of 25+ and co-morbidities like Type 2 Diabetes, Hypertension, Cardiovascular Disease, Osteoarthritis, Dyslipidaemia, Non-diabetic Hyperglycaemia, sleep apnoea
  • A BMI of 23+ and from Black African, African-Caribbean or South Asian population.

Additionally, they must:

  • Be aged 16 years or over
  • Live, work or study in Bedford Borough, Central Bedfordshire or Milton Keynes
  • Be registered with a GP in Bedford Borough, Central Bedfordshire or Milton Keynes.
Exclusion criteria

The programme is not suitable for individuals who:

  • Have an eating disorder
  • Have an underlying medical cause for obesity and would benefit from more intensive clinical management than a tier two service
  • Have significant complex or unstable co-morbidity, or have complex needs requiring specialist support, as identified by their GP or other healthcare professional.

Families healthy weight programme

Our family focussed services are for:

  • Children and young people aged 2 – 18 years
  • Those with a BMI over the 91st percentile (applicable from age 2 years).

And one of the following

  • Are a resident of Bedford Borough, Central Bedfordshire or Milton Keynes
  • Study in Bedford Borough, Central Bedfordshire or Milton Keynes.
